Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 17 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Home Care experience required License: Physical Therapist Professional license in Massachusetts Must-Have: - Ability to travel to cases as assigned - Weekend rotations once every 6 weeks Description: Provide physical therapy services to patients in their place of residence. Patients may have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with the patient, family, physician, and other allied health professionals to identify patient needs. Coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications. Adhere to agency policies, procedures, and standards of practice. Patients may range in age from birth to over 100 years old.
