Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Home Care experience License: Physical Therapist Professional license in Massachusetts Certifications: None specified Must-Have: - Experience specifically within a certified home health agency - Ability to travel to assigned cases Description: We are seeking a Physical Therapist to provide physical therapy services to patients in their place of residence. The ideal candidate will work with patients who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. The role involves collaborating with the patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Using thorough and timely electronic documentation and interdisciplinary communications is essential in accordance with agency policies, procedures, and standards of practice. Weekend rotations will be required approximately once every six weeks.
