Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8.5 hours Experience: Minimum 1 year in Home Care License: Physical Therapist Professional license in Massachusetts Certifications: None specified Must-Have: 1 year Certified Home Care experience, 1 year Case Management experience Description: Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with the patient, family, physician, and other allied health professionals to identify patient needs. Coordinate services to meet patient needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice. Weekend rotations are required once every 6 weeks. Ability to travel to assigned cases is necessary. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Start date is based on credentialing clearance. If not cleared by the posted start date, a rescheduled start date will be necessary.
