Key Responsibilities of the Physical Therapist
Conduct SOC (Start of Care), evaluations, reassessments, and discharges
Coordinate visit times with patients and manage daily schedules
Collaborate with PTAs for routine visits
Complete documentation in partner agency EMRs, including Pariox, Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
Requirements of the Physical Therapist
Valid Physical Therapy license
Experience in home health settings preferred
Proficiency with EMR systems
Ability to cover Saturday orthopedic visits
