Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 10 hours (08:00 - 18:30) Experience: Minimum 1 year required, with home health experience preferred License: State License Certifications: BLS, DPT Must-Have: Previous experience in charge or patient ratio experience is beneficial. Experience using Epic charting system is required. Travel experience is preferred. Description: We are seeking a home health physical therapist to provide care to patients in their homes. The therapist will be responsible for patient evaluations, reassessments, and discharges. Patients primarily have general weakness, back pain, and orthopedic conditions. Therapists will conduct home assessments and develop a plan of care tailored to the individual needs of the patient. The role requires participation in weekend duties on a rotating schedule, taking one weekend off every five weeks. The position involves travel to multiple counties, necessitating efficient time management skills. Candidates must be adaptable and able to work independently in various settings while managing a caseload. Experience in using assistive devices and adaptive equipment is essential. A commitment to patient education and care planning is required. Candidates should demonstrate strong assessment skills and the ability to identify environmental barriers for patients. The position may involve working with patients with chronic conditions, including COPD, CHF, and various neurological issues. Strong interpersonal and communication skills are a must, as the therapist will work with both patients and families.
