Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Days Hours per Shift: 40 Experience: Minimum 3 years License: Active license as a PT in the applicable state Certifications: Basic Life Support (BLS) certification is preferred Must-Have: Skilled nursing experience Description: The Physical Therapist partners with residents and other care providers to provide physical therapy services. The aim is to maximize residents' functional abilities and independence in their activities of daily living. Essential duties include performing evaluative and therapeutic procedures under the order of a physician. The Physical Therapist observes precautions, indications, and contraindications during treatment. Evaluates residents and establishes a therapeutic plan of care that includes functional goals. Freqency and duration of services and therapeutic modalities to be used are determined in this plan. Input from the resident and primary caregivers is sought regarding the therapeutic plan of care. This information is incorporated into the therapy goals to ensure personalized care. Consultation is provided to the referring physician and other team members regarding the plan of care. Participation in all relevant team meetings is required to support comprehensive care. Treatment effects are monitored and evaluated to progress the treatment program towards established goals. Modifications to the treatment plan are made as necessary based on resident progress. Documentation is completed in the Electronic Medical Record for all evaluative and therapeutic interactions. Ensures that documentation is accessible and clearly understood by all care team members. Recommendations for appropriate assistive devices are made to residents, with instructions for use provided. Education is offered to residents and/or caregivers regarding their care and treatment. The results of this teaching are documented for transparency and follow-up. Referrals are made to Physical Therapist Assistants as appropriate to facilitate care. Collaboration with other therapy professionals is essential to ensure optimal resident care. Understanding and compliance with reimbursement regulations such as Medicare are mandatory. Resident care documentation must adhere to Medicare, Medicaid, and Private Pay requirements. In-service education is provided regarding clinical knowledge and skills to other departments. Maintaining up-to-date clinical skills and knowledge through learning activities is expected. Awareness of professional issues is encouraged through membership in professional organizations. Performance issues should be reported to the Director. Mentorship is provided to new staff during orientation, ensuring completion of departmental procedures. Department cleanliness and maintenance are part of the responsibilities, including routine cleaning. Adherence to infection control procedures is essential in all activities. Maintaining required licenses, certifications, and competencies is a priority. Other duties consistent with this role may be assigned as needed. Working Conditions/Physical Demands: Ability to perform tasks involving a combination of sitting, standing, walking, bending, stooping, twisting, and reaching. Must be able to lift up to 50 pounds; above this weight requires assistance from another person. Frequent lifting or carrying up to 20 pounds is expected. Some stress may arise from the high level of responsibility for quality care.
