Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Physical Therapy Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 1 year of skilled home health experience License: Active license required Certifications: BCLS required Must-Have: Reliable transportation Description: Home Health Physical Therapist position available in Seaside, OR and surrounding areas. Start date is as soon as possible. Shift is 8 hours per day, 5 days a week. This facility does not accommodate 4x10 or 3x12 shifts. Patient ratios are 1:1, with potential visits to up to 6 patients per day. 30 productivity points are the standard. First-time travelers are welcome to apply. No weekend shifts are required. On-call duties are not required. Travelers are not expected to float between different units. Block scheduling is not available. Local candidates are accepted within a 50-75 mile radius. Pending licenses may be accepted. The facility can call off up to one shift per pay period. Travelers should have their own transportation. The system efficiently groups clients to minimize driving time. Occasional maximum driving radius is roughly 60 miles, resulting in about 60-90 minutes of total drive time per day. The facility requires primary series COVID vaccination and one booster. Familiarity with Oasis is necessary for this role. Business casual attire is acceptable, and scrubs may be worn but are not required. Charting is done using EPIC Remote Client for Home Health on a laptop and Rover on a phone app. No specific color for scrubs is designated. Mileage reimbursement follows IRS standard guidelines. References are required covering the last year of experience. All travelers are expected to float between facilities within the same state. Refusal to float may result in termination of the assignment. Travel-related communication devices will be supplied if necessary. Providers may submit their cell phone bills directly for reimbursement for work-related use. Modules related to training are non-billable and are completed before the start date.
