Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: 1 year of Skilled Home Health Experience required License: Pending license accepted Certifications: BCLS required Must-Have: OASIS experience required Description: Home Health Physical Therapist needed for East Portland and surrounding areas. Shift is 8-hour days. This facility cannot accommodate 4x10 or 3x12 shifts. Ratios are 1:1 with the possibility of visiting up to 6 patients per day. Productivity standard is 30 points per day. First-time travelers are accepted. No weekend requirements. On-call duties are not required. Travelers are expected to have their own transportation. The system is efficient in grouping clients, minimizing drive and windshield time. There may be an occasional maximum radius of 60 miles for daily driving time. Business casual attire is preferred; jeans are not allowed, and scrubs are permitted but not required. Charting is done using remote software and mobile applications. Mileage reimbursement follows IRS standard rates. The daily census averages between 5.5 and 6 points per day. Patient demographics include adults and geriatrics. Documented patient diagnoses include dementia, Parkinson’s, ALS, MS, orthopedic post-op, and post-stroke. Travelers are expected to float between facilities within the state as required. References will be required during the compliance process prior to the start. Travelers who refuse to float may face termination of the assignment. Communication devices may be supplied by the client if necessary.
