Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year of skilled home health experience required License: Must have a valid state license Certifications: Basic Life Support (BCLS) required Must-Have: Experience with Oasis documentation Description: Home Health Physical Therapist position available. This role requires a valid state license. The position involves visiting patients in their homes. The therapist will work primarily daytime hours. Productivity expectations include seeing up to 6 patients per day. A typical patient-to-therapist ratio is 1:1. First-time travelers are welcome to apply. No weekend work is required. Transportation is necessary as the therapist will travel to various patient homes. The facility works to minimize travel time between patient visits. The role requires full vaccination for COVID-19, including a booster. Experience with Oasis documentation is essential for this position. Business casual attire is expected, including the option to wear scrubs. The charting system used is EPIC for remote client management. The patient population includes adults and geriatrics. Common diagnoses include dementia, Parkinson's, and orthopedic post-operative cases. Additional skills required: lifting and transferring patients, scheduling appointments, and ordering supplies. All travelers are expected to float between facilities within the region. Refusal to float may result in termination of the assignment.
