Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8am to 4pm with 2-3 weekend days Experience: Minimum of 2 years License: Valid Physical Therapy license Certifications: APTA guidelines Must-Have: - Comprehensive assessment skills - Treatment planning and implementation experience - Coordination of patient care Description: Actively monitors and amends patient visit schedules to maximize productivity and patient needs. Takes action to account for new patients and changes in existing patient schedules, including rescheduling visits and communicating changes to supervisors. Performs comprehensive assessments of assigned patients, including psychosocial, educational, safety, and risk factor parameters. Develops plans of care to promote optimal functional independence and safety, including patient-specific goals. Plans for and provides skilled physical therapy treatment, assessing patient status and progress on each visit. Coordinates patient care with other clinical team members, ensuring optimal patient care and communication. Supervises and directs clinical activities of Physical Therapy Assistants, students, and home care aides. Completes required paperwork to ensure documentation reflects care provided and outcomes achieved. Ensures accurate charging and billing processes are executed, including necessary insurance forms. Participates in in-service training programs for various staff and community groups. Maintains personal knowledge and promotes standards in physical therapy. Engages in marketing and promotion of rehabilitation services. Carries out patient-related health assessments and procedures as needed. Communicates changes in patient care schedules to appropriate personnel promptly. Completes necessary documentation in accordance with guidelines and regulations. Works collaboratively with quality improvement staff to maximize accuracy of clinical notes. Follows established protocols for documentation processes. Accurately completes Agency, State, and Federal forms in a timely manner. Carries out visits efficiently and communicates scheduling information timely. Assesses patient needs for home care assistance and coordinates plans of care. Coordinates safe patient discharges and ensures all appropriate paperwork is processed. Transmits patient incident reports to quality improvement specialists as necessary. Participates in regular peer review audits and accepts feedback professionally.
