Radius rule: Traveler's permanent address must be 50+ miles outside home unit.

Shift & Scheduling
8hr Day Shift (0800-1700) - 40 hours/week.
6 visits/day; required 30 visit equivalents (VE)/week (cancelled appts not counted as VE).
No weekends, no call, no major holidays.
Schedule cycle: Every 13 weeks; RTO requires manager approval.
Teammates self-schedule patient visits the day prior.
Location & Travel
Travel throughout: Terrell, Sherrills Ford, Mount Holly, Maiden, Vale, Troutman, McAdenville, Lincolnton, High Shoals, Denver, Dallas, Cramerton, Belmont, and surrounding areas.
Average 60-80 miles/day (no max mileage); reimbursed weekly.
No company car; Samsung Note 9 phone provided for documentation/calls.
Proof of auto insurance required.
Patient Population
Chronic diseases; orthopedics/ortho surgery.
Experience & Qualifications
Minimum 1 year Home Health experience.
Accepts first-time travelers if seasoned in Home Health.
Required: NC PT State License, BLS, valid Driver's License + proof of insurance.
Key Skills: Evaluation, reassessment, discharges, PTA supervision.
Support & Team
Departmental leadership, schedular, clinical manager, clinical supervisor, peers/clinical team.

EPIC Rover (view-only).
Focura (upload discharge instructions, wound pics, etc.).
HCHB (Homecare Homebase).
Orientation
NEO + 5 days facility/unit orientation.
