This Home Health Physical Therapist opportunity is based in the Cedartown, Georgia area, a welcoming northwest Georgia community known for its small-town character, historic charm, and easy access to outdoor recreation. A typical day may include traveling between several patient homes in the Cedartown service area, completing initial assessments, revisits, and discharge visits. You will document visits using an electronic medical record system, communicate regularly with the broader care team, and participate in care coordination activities to ensure continuity and quality of care. Patient caseloads often focus on adult and geriatric patients with conditions such as post-orthopedic surgery recovery, stroke, cardiac and pulmonary diagnoses, deconditioning, and chronic disease management. Work schedules are generally structured around daytime hours, with flexibility for scheduling visits that best meet patient needs. Visit volumes and patient ratios are designed to allow for thorough, high-quality care and safe travel time between homes.
