Job Summary (List Format):
- Evaluate residents mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments. - Develop and implement individualized, person-centered physical therapy treatment plans to restore function, alleviate pain, and prevent disability. - Provide direct skilled therapy interventions, such as therapeutic exercises, gait training, and balance retraining. - Monitor resident progress, adjusting care plans as needed to achieve therapy goals. - Participate in interdisciplinary care team meetings and assist with discharge planning. - Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. - Supervise and guide PTAs and therapy aides in accordance with licensure requirements. - Accurately document treatment sessions, progress notes, and billing information in a timely manner. - Ensure compliance with all relevant state, federal, and facility documentation and practice standards. - Support quality assurance and performance improvement initiatives as part of the rehabilitation team. - Work primarily in a Skilled Nursing Facility (SNF)/Long-Term Care setting, requiring frequent standing, bending, and assisting with resident mobility. - Collaborate within a fast-paced, team-based rehabilitation environment.
