We are seeking a licensed Physical Therapist (PT) for a travel assignment in a skilled nursing and rehabilitation setting. The ideal candidate will evaluate, plan, and implement individualized treatment programs to improve patient mobility, function, and quality of life.
This role requires collaboration with an interdisciplinary care team to support patient recovery and rehabilitation goals.
Evaluate patients and develop individualized treatment plans
Implement therapeutic exercises and rehabilitation programs
Document patient progress and update care plans as needed
Work closely with physicians, nurses, and therapy staff
Educate patients and families on therapy goals and exercises
Ensure compliance with facility and clinical standards
Support patient mobility, strength, and functional independence

Active Physical Therapist (PT) license required (Maine or compact/eligible per facility rules)
BLS certification preferred
Experience in skilled nursing or rehab setting preferred
