Job Title: Physical Therapist Profession: Physical Therapist (PT) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 1 year of PT experience required. License: Must have a Massachusetts State License. Certifications: N/A Must-Have: Long Term Care or Rehabilitation experience required. Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including, but not limited to: manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices. Provides wound care as needed. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings. Engages in facility and company required training, in-services, and conferences. Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to professional practice regulations and ethics throughout employment as a Physical Therapist.
