Job Summary Physical Therapist (PT) in Skilled Nursing Facility
- Evaluate residents mobility, functional abilities, and limitations through initial and ongoing assessments.
- Develop and implement individualized, person-centered physical therapy treatment plans.
- Deliver direct therapy interventions (e.g., therapeutic exercises, gait/balance training, use of modalities).
- Monitor resident progress, adjusting treatment plans to achieve therapy goals.
- Collaborate with interdisciplinary care team and participate in discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides.
- Complete timely and accurate documentation of treatment sessions and billing.
- Ensure compliance with state, federal, and facility standards and regulations.
- Support quality assurance and performance improvement initiatives.
- Work primarily in a skilled nursing facility/long-term care setting.
- Environment requires frequent standing, bending, lifting, and assisting residents.
- Operate as part of a fast-paced, collaborative rehabilitation team.W
