The facility serves a diverse post-acute caseload, including patients recovering from orthopedic surgeries, fractures, strokes, deconditioning, cardiopulmonary events, and other complex medical conditions that benefit from intensive rehabilitation. Your typical day will include evaluating new admissions, developing individualized plans of care, and providing one-on-one and small-group treatments designed to restore mobility, improve balance, increase strength and endurance, and maximize independence in activities of daily living. You will perform gait training, transfer training, therapeutic exercise, neuromuscular re-education, and patient and family education, while closely monitoring functional progress and adjusting treatment plans as needed. You will collaborate closely with nursing and the broader care team to coordinate safe discharge planning, including recommendations for equipment, home exercise programs, and support services. Documentation is completed in an electronic medical record system, with an emphasis on accurate functional reporting and outcome measures that reflect patient progress and regulatory requirements. Depending on the unit, patient caseloads are designed to support quality, individualized care and may range through a typical skilled rehab PT schedule over day or evening shifts. Shifts are most commonly daytime hours with potential for some weekend or holiday rotation to support continuous rehab services. The environment is professional yet welcoming, with a focus on mentorship, collaboration, and ongoing skill development.
