Job Summary (List Format):
- Evaluate residents mobility, strength, gait, balance, and functional limitations through initial and ongoing assessments. - Develop and implement individualized, person-centered treatment plans to restore or enhance physical function, reduce pain, and prevent disability. - Provide direct physical therapy interventions, including therapeutic exercises, gait and balance training, and use of physical modalities. - Monitor residents progress, adjust treatment plans as needed, and ensure therapy goals are met. - Collaborate with an interdisciplinary care team and participate in care planning and discharge processes. - Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. - Supervise and mentor Physical Therapist Assistants (PTAs) and therapy aides, following licensure guidelines. - Accurately document all treatment sessions, progress notes, and billing information in compliance with state, federal, and facility standards. - Ensure adherence to facility policies, physician orders, and regulatory requirements. - Contribute to quality assurance and performance improvement initiatives within the rehabilitation team. - Work in a skilled nursing facility/long-term care environment, often requiring physical activity such as standing, bending, and assisting residents with mobility. - Function both independently and as part of a fast-paced, team-based environment.
