Job Summary (List Format):

- Evaluate residents mobility, strength, balance, gait, and functional limitations through initial and ongoing assessments.
- Develop and implement individualized, person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
- Provide direct skilled physical therapy interventions, including therapeutic exercises, gait and balance training, and use of physical modalities.
- Monitor resident progress and adjust treatment plans as needed to achieve therapy goals.
- Participate actively in interdisciplinary care team meetings and assist with discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety techniques to promote independence.
- Supervise and support Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure regulations.
- Accurately document treatment sessions, progress notes, and billing information in compliance with facility, state, and federal standards.
- Ensure adherence to all relevant documentation, practice, and regulatory requirements.
- Support quality assurance and performance improvement initiatives within the rehabilitation team.
- Work collaboratively in a skilled nursing facility/long-term care environment, which may involve frequent standing, bending, lifting, and assisting residents with mobility.
