Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum of 1 year of Physical Therapy experience required. License: Massachusetts State License required. Must-Have: - Long Term Care or Rehabilitation experience. - Effective communication skills with patients, families, and healthcare team members. - Ability to assess patient progress and modify treatment plans accordingly. Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including but not limited to: manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services based on patient evaluation and makes referrals in relation to outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Communicates effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Participates in required trainings, in-services, and conferences. Obtains any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to professional practice standards and ethics during the course of practice as a Physical Therapist.
