Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: Minimum of 1 year Home Health experience License: NC State License Certifications: BLS Must-Have: HCHB Homecare Homebase, Minimum of 1 year Home Health experience Description: Home Health position requires a physical therapist. The role involves completing approximately 6 patient visits a day. There is no maximum mileage to drive, with an average of 60-80 miles per day. Mileage reimbursement is provided weekly. The candidate must have experience with chronic diseases and orthopedic diagnoses. Teammates will travel to various areas within the designated coverage zone. Candidates must possess a driver's license and proof of insurance. Key skills required include evaluation, reassessment, discharges, and supervision of PTAs. Support is available from departmental leadership, schedulers, and clinical managers. Technology used includes EPIC Rover view and Focura for documentation. The position involves scheduling visits with patients one day in advance. Cancelled appointments do not count as visit equivalents. Required visits per week are specified. A cell phone for documentation and communication will be provided. The work culture is team-oriented. Weekend rotations and major holidays are not included in the schedule. Requests for time off require manager approval. Schedule cycles occur every 13 weeks.
