Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 8-hour Day Shift Hours per Shift: 0800 - 1700 Experience: A minimum of 1 year of experience required License: State Licensure required Certifications: BLS required Must-Have: Charting experience with Homecare Homebase is required. Description: Department Information: Approximately 6 visits per day. Patient types include chronic diseases, wound care, IV, pleurx, and wound vac. Required skills include IV, pleurx, wound vac, wound care, assessment, and disease teaching. Experience with geriatric/adult chronic disease management and maintenance notes is essential. Support will be available from departmental leadership. The technology used includes Samsung note 9 phones. EMR system is Homecare Homebase, which is required. OASIS experience is a requirement. Weekend rotation is not required. On-call duties are not required unless specifically requested. Scrub color is grey.
