Job Title: Physical Therapist (PT) Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 year of adult home health experience required License: Must have a current license in hand Certifications: CPR, BLS Must-Have: - Reliable transportation - Current driver's license - Agency-required liability insurance Description: Home visits required for therapy services. Periodic office meetings will be held. Must travel up to 75 miles from home clinic. Mileage will be reimbursed at the IRS standard rate. Contractor will see an average of 5-10 patients per day. Each patient equates to different points based on visit type and documentation activities. A total of 30 points per week is expected. Patient types may include start of care and follow-ups. Must have recent home health experience. Weekend requirements vary; travelers should discuss during the interview. There is a possibility of on-call duties, depending on location and job specifics. Documentation must be completed within 24 hours of each patient visit. Delayed documentation that impacts billing and patient care may lead to cancellations. There are no guaranteed hours with this contract. Contractors must meet weekly productivity expectations of 30 points. This may result in a rolling requirement based on patient census, with opportunities to pick up additional hours if needed. Must comfortably accept all patients assigned to the schedule without decline, regardless of travel distance.
