Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Home health experience required. Must be proficient in home health evaluation, treatment planning, patient and caregiver education, and documentation of skilled services.
• Ability to meet productivity expectations: minimum of 35 points per week.
• Reliable transportation and ability to travel within assigned territory (Territory: Purple) to visit patient homes.
• Previous experience managing a caseload in home health settings preferred.
