Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 2 years in Long Term Care setting License: TX or compact license required Certifications: Not specified Must-Have: 2 years PT experience in LTC setting, TX or compact license Description: The physical therapist will accurately assess, diagnose, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The therapist works with the individual to prevent the loss of mobility. Essential functions and responsibilities include performing patient evaluations by obtaining history, performing relevant system reviews, and selecting and administering specific tests and measures to gather data. The therapist establishes an individualized plan of care and selects treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders. They conduct ongoing reassessment of the patient and their response to treatment. The therapist initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment. They supervise and delegate to ancillary staff and students in accordance with state laws and department policy. The therapist completes legible, accurate, and clear documentation within the established timeframe as identified in department policy and procedure and in accordance with accreditation requirements and insurance regulations. They communicate with the patient, family, caregivers, significant others, and members of the healthcare team to promote maximum benefit from care. The therapist utilizes educational strategies to assess the needs of the patient, family, caregiver, or significant other and provides education to meet those needs. They participate in professional growth and development through educational programs, including in-service meetings, workshops, and by reviewing current literature. The therapist actively participates in quality improvement activities. They provide a safe environment for patients, visitors, and staff. They comply with appropriate and approved safety standards. The therapist functions within the scope of practice as defined by the state’s practice act. Other related duties may be required and directed.
