Job Title: Physical Therapist (PT) - Home Health Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Must have reliable transportation Must have a current driver’s license Must have agency-required liability insurance Must be willing to travel up to 75 miles from home Description: This position involves providing therapy services through home visits. Periodic office meetings are required. The average number of patients per day is 5-6. Contractors will be assigned between 5-10 patients per day, equating to roughly 6-8 points. There are no guaranteed hours, but opportunities for additional hours may arise. Types of cases include Start of Care and Follow Ups. Recent experience in Home Health is required; first-timers will not be accepted. Weekend requirements may vary by location and profession. Traveling from patient home to patient home is compensated per IRS standard rates. The commute to and from the first and last patient are not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted unless it takes a few days to obtain. The contractor must be open to traveling up to 75 miles away from the assigned care center. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in a cancellation. Clinicians must accept every patient on their schedule regardless of distance. All travelers must be proficient in the required EMR system and have used it within the last 6 months, or complete a retraining course. No contract hours are guaranteed.
