Looking for a change of scenery and a chance to help others. Provide skilled physical therapy in a home health setting for patients across the lifespan. Plan, implement, and evaluate individualized plans of care based on medical referral and clinical evaluation. Collaborate with an interdisciplinary team to restore or maintain patient function, instruct patients and caregivers in safe transfer and equipment use, and document clinical progress and orders in the medical record.


Minimum Qualifications

  • Bachelor of Science in Physical Therapy from an APTA-approved program or equivalent degree.
  • Current, active Washington State license as a Physical Therapist required to start the assignment.
  • Proof of current CPR certification required.
  • Current valid driver’s license, reliable transportation, and insurance required.
  • 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
  • Ability to perform physical demands of home health visits including lifting, bending, pushing, pulling, prolonged standing and walking.
  • Ability to read, write, and speak fluent English.
  • Ability to work a flexible schedule and travel locally.
  • Knowledge of durable medical equipment and ability to order/adapt equipment for home environments.
  • Must meet mandatory continuing education requirements.


Preferred Qualifications

  • Previous home health experience preferred.

Provide evidence-based interventions for neuromuscular, neurological, and orthopedic diagnoses. Document and revise plans of care, progress notes, and phone orders. Educate patients and families on therapeutic procedures and safe use of assistive devices. Apply infection control and universal precautions. Supervise other team members when applicable. Maintain professional growth and competency while contributing to improved patient outcomes in community-based care.
