License: Must hold a Massachusetts State License.
Certifications: LTC or Rehab experience preferred; New grads welcome with Level II experience.
Must-Have:
- Effective screening, evaluation, and rehabilitation of patients with physical functioning disorders and/or wound care.
- Development of appropriate plan of care following all regulatory and clinical practice standards.
- Utilization of standardized assessments and evidence-based practice to support clinical interventions.
- Administration of various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assessment of patient progress and adjustment of treatments as necessary while maintaining clinical records as per regulatory and clinical practice requirements.
- Evaluation of treatment outcomes for effectiveness and modification of services in relation to patient evaluations.
- Provision of consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Effective communication with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provision of comprehensive discharge summaries of services provided in accordance with regulatory and clinical requirements.
- Documentation of all regulatory and clinically required information in the electronic documentation system and maintenance of patient records according to company policy.
- Accurate recording of treatment minutes for services provided per regulatory and clinical requirements.
- Participation in required meetings, including patient care conferences and rehabilitation meetings.
- Participation in facility and company-required trainings, in-services, and conferences.
- Ability to obtain necessary durable medical equipment or supplies per regulatory and clinical practice requirements.
- Supervision and support of physical therapist assistants, rehabilitation aides, and/or students.
- Adherence to practice acts, code of conduct, and ethics during professional practice as a physical therapist.
