Job Title: Physical Therapist Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent home health experience Description: Home visits are required for this position. Periodic office meetings will be scheduled. Candidates must have reliable transportation, a current driver's license, and agency-required liability insurance. The average number of patients per day is 5-6. Contractors are expected to be assigned between 5-10 patients daily, which equates to roughly 6-8 points. Guaranteed hours are not provided. If patient census is low and impact financial needs, contractors should communicate with management for options. Types of cases include Start of Care and Follow-Ups. First-time home health experiences are not accepted. Weekend requirements vary by location and profession. On-call expectations may vary by job or location and should be discussed during the interview. Traveling up to 75 miles from the home clinic is required. Mileage will be reimbursed at the current standard IRS rate. Traveling from patient home to patient home is compensated. Commutes to and from the first and last patient are not billable. Local candidates are accepted at the same rate. Pending licensure is not acceptable, except in cases where the license takes a few days to obtain. The initial assignment is expected to last for 13 weeks. The typical schedule is Monday through Friday from 8 AM to 5 PM. Weekend rotations may vary based on location. The number of miles traveled per week will be around 200. Consultants are expected to meet weekly productivity expectations of 30 points. The EMR system used is HomeCare HomeBase. Experience in home health is required.
