As a Home Health PT in York, you will provide one-on-one rehabilitative care to patients in the comfort of their homes, helping them regain independence, mobility, and safety. You will perform comprehensive assessments, including strength and balance testing, gait and transfer analysis, home safety evaluations, and fall-risk screening. A typical day involves traveling within a defined service area around York to complete scheduled patient visits, with time allotted for documentation and care coordination. Daily visit volumes are designed to support quality, patient-centered care and reasonable travel time, often ranging from 5 to 7 visits per day depending on patient needs and geography. The role requires careful attention to documentation within the agency’s EMR system to meet regulatory, accreditation, and quality standards in home health.
