Key Responsibilities of the Physical Therapist
  • Evaluate and treat patients based upon physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans for maximum independence/function
  • Regularly re-assess effectiveness of treatment plans and modify as needed
  • Communicate patient needs and progress to the treatment team and family members
  • Consult with medical providers regarding evaluation, treatment, progress, and discharge
  • Recommend and assist in securing special adaptive equipment for patients
  • Maintain and submit evaluation, treatment, and billing documentation as required
  • Adhere to standards of professional practice and applicable laws
  • Supervise Physical Therapist Assistants and Aides in patient care activities
  • Attend internal staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist
  • KY PT License, Compact License Accepted
  • Experience with Home Care Home Based and Medicare Oasis
