Job Title: Physical Therapist Profession: Physical Therapist (PT) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1 year of PT experience required; new graduates with Level II experience considered License: Missouri State License required Must-Have: - LTC or rehab experience required Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summary of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided in compliance with all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Engages in facility and company required trainings, in-services, and conferences. Obtains any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and students. Adheres to professional practice ethics and codes of conduct during practice as a Physical Therapist.
