Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours, M-F (Times are flexible as long as consistent and communicated with DOR) Experience: 1 year of PT experience, LTC or Rehab experience License: MO State License Certifications: None specified Must-Have: New grads are okay with Level II experience Description: This position follows the clinical interview process. Candidates can expect outreach within two hours via phone and text, where applicable. Decisions are typically made the same day or by the next business day. Applicants who are looking for more than 7 days off or who have negative background or employment history are not eligible for the same processing timeline. If a new grad, the candidate will not go through the clinical interview process but will be presented for review. Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including, but not limited to, manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness and modifies services in relation to patient evaluation. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Participates in required trainings, in-services, and conferences. Coordinates obtaining necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
