Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum of 1 year of PT experience preferred. New graduates are encouraged to apply with appropriate Level II experience. License: OK State License required. Must-Have: - LTC or rehab experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in clinical records per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summary of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences and utilization review meetings. Participates in facility and company required trainings, in-services, and conferences. Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support physical therapist assistants and/or rehabilitation aides. Adheres to professional practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
