Job Title: Physical Therapist Profession: Therapy Specialty: Physical Therapist Duration: 14 weeks Shift: 5x8 Hours per Shift: Monday - Friday 8am - 4:30pm EST; no weekends - some evenings Experience: Minimum 1 year of geriatric rehabilitation experience License: Client Physical Therapist license Certifications: None specified Must-Have: Ability to read, analyze, and interpret common journals, reports, and legal documents Description: Perform comprehensive evaluations within twenty-four hours of referral. Plan and implement an appropriate treatment program with measurable, objective, realistic goals based upon evaluation data. Complete necessary documents following evaluation, including contracture evaluation, balance sheet, and physician certification for restorative treatment and therapy attendance sheet. Demonstrate safe resident care techniques consistently. Maintain legible, complete records of resident evaluations, progress notes, and discharge notes in the computer system and the residents' charts. Work as an integral part of the rehabilitation team to meet resident needs. This includes working within the departmental team as well as resident/family instruction and contact with involved health professionals. Perform resident treatments as outlined in evaluation, with appropriate changes as residents' status changes. Treatments may include but are not limited to transfer training, bed mobility, ambulation, exercises, massage, and use of electrical or battery-operated modalities. Attend scheduled family and clinical meetings. Assist with the supervision and training of new personnel, assistants, aides, students, and volunteers. Attend continuing education courses/seminars as appropriate, with follow-up in-service training to the department. Perform other duties as assigned. Must be able to apply basic mathematical concepts and practical problem-solving skills. Ability to interpret a variety of instructions furnished in written, oral, diagram, or scheduled form. May be required to lift and/or move up to 100 pounds. Specific vision abilities required include close/distant vision, color/peripheral vision, and depth perception. Ensure personal safety while monitoring the work environment and making necessary improvements to ensure the safety of others.
