Outpatient clinic experience

We give 2 full days of orientation and training and have each new hire or traveler. I spend a few hours going through orientation and detailed walkthrough of EPIC. I then have them paired with a therapist to learn EPIC and work as a scribe. All documentation must be done the day of service, THIS IS A MUST. They do not start seeing patients until day 3. The first week, they have 50% caseload (one patient scheduled every hour). By the middle of the second week, we spread that out to two patients and a blocked spot. By the middle of the third week, they have three consecutive patients, then a blocked spot. By week 4, they have a full schedule, without any double books. I usually add double books by week 5. Because I knew Daniel was struggling with caseload and documentation, I did not even add double books until Week 6 and only partially at that. This, his seventh week, is really his first true full caseload. I know that I am far more conservative at ramping up clinicians than I would say the majority of other leaders and clinics are. I would say most have their clinicians ramp up to a full caseload by the end of week 3, week 4 max. I try to be very understanding while still trying to hold everyone accountable to expectations.

Documentation and workflow is the biggest pain point for all therapists, and is not unique to travelers or new hires. It is constantly a struggle to manage, but unfortunately that is the way healthcare and outpatient rehab has gone. Insurance reimbursements are less, we are required to see more patients to offset that, and documentation requirements continue to increase to justify care and reimbursement. We would all love to be one on one with patients every hour, but that is just not realistic or feasible, and that is anywhere, not just Sentara.
