Physical Therapist - Home Health - (PT - HH)

Pay Rate: $2429.00 - $2503.00
Travel PT Home Health jobs in Hilo, HI are available for 13-week assignments with Monday through Friday 8-hour day shifts, totaling 40 hours per week. You must have a current Hawaii PT license, home health experience, and proficiency with Homecare Home Base software.
Your responsibilities include evaluating patients in their homes, developing individualized treatment plans, implementing therapeutic interventions, educating patients and caregivers, and documenting visits. You will work 25-30 points per week and collaborate with a multidisciplinary team to optimize patient outcomes
