**Job Title: Home Health Physical Therapist Assistant**
**Location:** Concord, CA
**Role Overview:**
Provide essential physical therapy services to the geriatric population in their homes. Collaborate with healthcare professionals to ensure comprehensive care.
**Responsibilities:**
• Deliver quality therapy to home•based patients.
• Work with RNs and OT/PTs for integrated care.
• Manage a caseload of 6 patients daily.
• 13•week contract, Monday•Friday, 8:00 AM • 5:30 PM.
**Qualifications:**
• CA PTA license.
• BLS certification.
• 1 year of home health experience.
• Oasis/HCHB familiarity preferred; training available.
**Additional Info:**
• No on•call/holiday work.
• Flexible scrub color.
• Includes HR, HCHB training, and mentorship.
