Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1 year of PT experience or Level II experience License: Massachusetts State License Certifications: N/A Must-Have: LTC or Rehab experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifications to services are made in relation to patient evaluation, and referrals are made based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided in compliance with all regulatory and clinical requirements. Participates in required meetings, including patient care conferences, utilization reviews, and rehabilitation meetings. Engages in facility and company required trainings, in-services, and conferences. Able to obtain any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to regulatory practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist.
