Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 30 guaranteed hours Experience: Minimum of 2 years clinical experience preferred; home health experience a plus but not required License: Currently licensed as a Physical Therapist in the state of practice Certifications: Current CPR certification Must-Have: Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation Ability to exercise sound clinical judgment and work independently in varied patient care settings Strong communication and interpersonal skills with sensitivity to diverse backgrounds Basic computer proficiency; willingness to learn EMR systems and OASIS documentation Reliable transportation, valid driver's license, and auto insurance for home visits Description: Provide physical therapy services within the legal scope of practice for the state of practice, following all licensure requirements. Maintain valid licensure and certifications as required by state and federal regulations. Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and agency standards. Develop, implement, and update individualized plans of care aligned with physician orders and patient needs. Deliver skilled interventions and maintenance therapy programs as permitted by state-defined scope of practice. Directly supervise Physical Therapist Assistants and Home Health Aides in compliance with state practice acts. Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress. Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety. Prepare and submit assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes. Appropriately apply ICD-10 coding for accurate and compliant documentation. Present in-service programs periodically to agency staff and contribute to staff education initiatives. Actively engage in Quality Assessment and Performance Improvement activities, supporting continuous compliance and clinical excellence. Provide care and services in accordance with infection control, safety, and risk prevention protocols. Accurately document clinical interventions, care coordination, and communication in a timely manner. Participate in interdisciplinary meetings, case reviews, and continuing education opportunities. Maintain compliance with privacy and confidentiality standards. Represent the organization positively and professionally in patients’ homes and community settings. Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities. Provide courteous, timely, and professional customer service to patients, families, and partners. Exhibit adaptability to changing responsibilities and flexibility in completing assignments. Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms. Adhere to all organizational policies, including job descriptions and the Employee Handbook. Communicate and interact professionally and respectfully with others to support team goals.
