Job Details:
  • Base Hours = 40/week
  • Pay Rate = $21.50/hr (Travel), $1500 stipend
  • Shift Hours = 0800 - 1830
  • Assignment Length = 13 Weeks

SnapCare is seeking a dedicated and compassionate Physical Therapist (PT) to evaluate, diagnose, and treat patients with physical limitations, injuries, or disabilities to improve mobility, reduce pain, and restore functional independence. The Physical Therapist will develop individualized treatment plans, provide therapeutic interventions, and collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.

Qualifications:
  • Active state licensure as a Physical Therapist.
  • Current CPR/BLS certification preferred.
  • Previous clinical experience in inpatient, outpatient, rehabilitation, home health, or skilled nursing settings preferred.
  • Strong knowledge of physical therapy techniques, rehabilitation practices, and patient care standards.

Responsibilities:
  • Evaluate patients' physical conditions, functional abilities, and medical histories to develop individualized treatment plans.
  • Design and implement rehabilitation programs to improve mobility, strength, balance, coordination, and pain management.
  • Provide therapeutic exercises, manual therapy, stretching, and mobility training.
  • Educate patients and caregivers on treatment plans, exercises, injury prevention, and home care techniques.
  • Monitor patient progress and modify treatment plans as needed to achieve rehabilitation goals.
  • Maintain accurate and timely patient documentation, including assessments, progress notes, and discharge summaries.
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals regarding patient care.
  • Utilize medical equipment and assistive devices to support patient rehabilitation.
  • Ensure compliance with healthcare regulations, facility policies, and safety standards.
  • Promote patient independence and quality of life through effective therapy interventions.
