Job Title: Physical Therapist Profession: PT (Physical Therapist) Specialty: PT (Physical Therapy) Duration: 13 weeks Shift: 8H Days M-F, rotating weekends Hours per Shift: 40 hours per week Experience: Minimum 1 year required License: License to practice required Certifications: BLS Certification required Must-Have: Preference for candidates with experience; no new grads Description: Department: Physical Therapy The position involves providing home health physical therapy. Daily responsibilities include start of care, recertifications, evaluations, and visits. Common diagnoses include CHF, ortho issues, and pneumonia. Staff will be required to cover various patient areas as needed. A clinical manager or educator will be available for support during business hours. Work schedule includes five 8-hour days, with weekend shifts on a rotating basis. Candidates will be included in holiday rotation as necessary. Scrubs or business casual attire is acceptable. New hires will receive two weeks of orientation with a work coach. Cancellation policy allows for up to four shifts to be canceled during the assignment. Charting will be done using Homecare Homebase (HCHB).
