Minimum Requirements

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree approved by the APTA.
• Current state license as a Physical Therapist in Kentucky required to start the assignment.
• Proof of current CPR certification required.
• Current driver’s license, reliable transportation, and vehicle insurance required.
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Experience with durable medical equipment and ordering rehabilitative equipment required.
• Ability to perform patient lifting, transfers, prolonged standing and walking, and manual dexterity consistent with home health duties.
• Ability to work a flexible schedule and travel locally required.
• Must read, write, and speak fluent English.
• Previous home care experience preferred.
