Join a home health team in Rolla, MO as a Physical Therapist. Provide skilled physical therapy services to patients across the lifespan in their homes. Plan, implement, and evaluate individualized plans of care based on physician referrals and clinical assessments. Collaborate with interdisciplinary teams, provide patient and family education, recommend and adapt durable medical equipment, and document progress and plan of care according to agency and regulatory requirements. This role requires travel within the local service area and physical activity including lifting, bending, and prolonged standing.


Minimum Qualifications

  • Current State License as a Physical Therapist, required to start assignment in Missouri.
  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree, required.
  • Proof of current CPR certification, required.
  • Current driver’s license, reliable transportation, and vehicle insurance, required.
  • 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting, required.
  • Ability to perform frequent standing, walking, lifting, bending, pushing and pulling in home environments.
  • Ability to travel locally and work a flexible schedule.
  • Must read, write, and speak fluent English.
  • Client requirement: documentation of Plans of Care, progress notes, and phone orders according to agency protocols, required.
  • Client requirement: ability to order and adapt rehabilitative equipment for home use, required.


Preferred Qualifications

  • Previous home care experience, preferred.
  • Knowledge of durable medical equipment, preferred.


Assignment Details

  • Local travel within service area is required.
