Minimum Requirements

• Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent bachelor’s, master’s, or doctoral degree in Physical Therapy.
• Current Kentucky Physical Therapist license required prior to start.
• Proof of current CPR certification required.
• Current driver’s license, reliable transportation, and vehicle insurance required.
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Previous home health experience preferred.
• Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams.
• Ability to perform transfers, instruct on durable medical equipment (walkers, wheelchairs, canes, braces, prosthetics), and order/adapt rehabilitative equipment for home environments.
• Ability to do heavy lifting, bending, pulling, pushing, prolonged standing and walking required.
• Ability to work a flexible schedule and travel locally.
• Must read, write, and speak fluent English.
