Minimum Requirements
- Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent bachelor’s, master’s, or doctoral degree in Physical Therapy.
- Current Kentucky Physical Therapist license required prior to start.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and vehicle insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home health experience preferred.
- Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams.
- Ability to perform transfers, instruct on durable medical equipment (walkers, wheelchairs, canes, braces, prosthetics), and order/adapt rehabilitative equipment for home environments.
- Ability to do heavy lifting, bending, pulling, pushing, prolonged standing and walking required.
- Ability to work a flexible schedule and travel locally.
- Must read, write, and speak fluent English.
