Minimum Requirements

  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree from an APTA-accredited school. Required.
  • Current State License as a Physical Therapist in Kentucky is required to start the assignment.
  • Proof of current CPR certification is required.
  • Current driver’s license, reliable transportation, and insurance are required.
  • 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting is required.
  • Previous home care experience is preferred.
  • Ability to perform patient assessments, prepare and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams as part of clinical practice.
  • Ability to safely instruct patients and families on transfers and durable medical equipment (walkers, wheelchairs, canes, braces, prosthetics).
  • Ability to perform prolonged standing, walking, lifting, bending, pulling, and pushing required for home health settings.
  • Must read, write, and speak fluent English.
  • Meets mandatory continuing education requirements. Preferred: demonstrable commitment to professional growth and competency.
