Serves four counties (Noble, Lagrange, Steuben, DeKalb), primarily rural with mid-sized towns. Shift M-F 8-5pm typically. Facility uses point system equivalent to 4-6 patient visits daily. Tablet used to track time and visits. Business casual or Hunter green scrubs with white or black top. Common cases: elderly falls, fractures. Must do Oasis Assessments and Start of Care cases.
