You will travel between patient homes, often driving scenic country routes and working with a diverse patient population, primarily adults and older adults recovering from illness, surgery, injury, or managing chronic conditions. You will coordinate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides to ensure cohesive, patient-centered care. A typical day often includes morning and afternoon home visits, completion of EMR documentation, communication with physicians and care coordinators, and regular phone or virtual check-ins with your home health team. You will have the opportunity to exercise a high level of clinical autonomy, while still benefiting from a supportive, collaborative structure and clinical leadership. The environment is well-suited for clinicians who value flexibility, independence, and the ability to see the full impact of their work over time. Shifts are generally daytime hours on weekdays, with some agencies requiring occasional weekend or on-call coverage depending on patient needs. Your schedule may include a mix of evaluation visits, routine follow-up visits, and re-assessments based on regulatory guidelines and the progression of your patients.
