Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health PT Duration: 13 weeks Shift: 8H Days Hours per Shift: 8:30 AM - 5:00 PM, typically one Saturday a month Experience: Minimum of 2 years as a Physical Therapist in an acute/home care/rehab setting License: Required Certifications: BLS Certification Must-Have: Good oral and written communication skills; strong organizational skills Description: The position involves providing physical therapy services to patients/clients according to a written physician's plan of care. Responsibilities include assessing, evaluating, and updating therapeutic, rehabilitative, and functional statuses. The role includes developing a total plan of care and evaluating the home environment. The physical therapist assesses muscle strength, mobility, gait, and range of motion to determine rehabilitation potential. Directing physical therapy treatment is a key responsibility. Instructing patients, clients, and families in the use and care of therapeutic appliances is essential. The physical therapist determines priority needs for physical therapy. It is important to report the patient's reaction to treatment or changes in condition to the physician. Initiating physical therapy programs and instructing other personnel and/or family members in certain phases of therapy is required. Training patients in the use of prosthetic devices may be necessary. Supervision of homemakers and aides participating in the patient's rehabilitation regime is part of the role. Identifying needs for additional home health services and making referrals is important. The position requires preparing and submitting clinical and progress summaries based on the attainment of therapeutic goals. Observing, recording, and reporting information on the patient's condition is mandatory. Providing physical therapy consultation to families or caregivers when indicated is also part of the job. Participation in peer consultation processes and ongoing staff competency assessments is required. Compliance with regulations and licensure requirements related to the position is essential. Other duties may be assigned as needed. Miles traveled for work are billable according to GSA guidelines, with specific documentation required. Accrued mileage begins after departing from the first assignment. Travel to the first visit does not qualify for reimbursement. Cancellation policy allows for a limited number of shifts to be canceled during the assignment. The charting system used is Homecare Homebase.
