Job Title: Physical Therapist Profession: PT Specialty: Physical Therapy - Post Acute Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years License: Required Certifications: BLS (AHA) Must-Have: Able to work independently and perform transfers. Excellent communication skills. Ability to work closely with an interdisciplinary team. Description: Position involves working primarily in a skilled nursing facility. Additional training to float to an inpatient acute care hospital may be required. Average number of patients treated per day is 8-9, focused on post-acute care. Common diagnoses include total joint replacements, stroke, deconditioning, pneumonia, and balance difficulties associated with general medical conditions. Patient population typically consists of geriatric individuals with chronic conditions and includes orthopedic and neurological patients. Floor-specific orientation provided. Charting system utilized is Cerner. No weekend requirement or holiday shifts expected. Cancellation policy allows for cancellation of up to 4 shifts per 13-week assignment.
